#1 APRIL MENU Lake Dow Children's Academy is a proud participant of the USDA Child and Adult Care Food Program. We serve only healthy and nutritious meals and snacks that meet or exceed USDA food pyramid guidelines. If you have a suggestion for a healthy meal or snack, please email Mrs. Michelle!

#2 TUITION RATES Here you will find information on registration fees, tuition fees, late payment fees, late pickup fees and more.

#3 PARENT HANDBOOK Our Parent Handbook is designed to aid the parent in learning the policies and procedures of our center. You will find information on hours of operation, tuition payments and fees, holidays, inclement weather procedures, meals and snacks, curriculum, field trips, etc.

#4 ENROLLMENT FORMS All new enrollees must fill out and turn in enrollment forms prior to admission to our center. Our enrollment packet includes parent and child information, authorized emergency contacts and pick-ups, medical authorization, discipline plan, etc.

#5 INFANT FEEDING PLAN Bright from the Start requires our center to obtain a signed written feeding plan for all children less than one year of age. This form aids the teacher in knowing nap schedules, feeding schedules, whether the child likes their bottles warm or cold, etc.

#6 SAFE SLEEP PRACTICES POLICY When enrolling an infant, the parent must read, sign and return the Safe Sleep Practices Policy form to the center prior to enrollment. This form notifies the parent of the guidelines for infants that the center will follow.

#7 INFANT AFFIDAVIT As a participant of the USDA Child and Adult Care Food Program, we are required to offer to provide formula and infant cereal to your infant. This form must be filled out and kept on file along with the Infant Feeding Plan.

#8 AUTHORIZATION FOR MEDICATION All medication, both prescription and non-prescription, must have an Authorization for Medication form filled out and signed by a parent in order for our staff to administer it to your child. Please be specific when listing times the medication should be given to your child. Terms such as "every 4 hours" and "as needed" are not allowed. Specific times must be listed such as "10am and 2pm". This form must be filled out and signed by a parent any time a child requires medication while attending our center.
